Regulation: 511-6-1.03(3)(b) - certification requirements/exemptions (pf)
Observed an expired CFSM certificate for the PIC, and no other valid and available CFSM certifications available at the establishment. CA: Certified Food Safety Managers must be designated to one food service establishment only and maintain and renew certification in accordance with the requirements of the examination taken. PIC assured he has signed up for class and will be taking the exam soon. Will provide picture proof once CFSM is received.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed dark matter on the soda dispenser nozzle in the bar area. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: PIC voluntarily cleaned and sanitized the nozzle.

Observed metal shelves in a walk-in cooler rusting. CA: Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.
Discussed with PIC to replace the shelving if the shelves cannot be resurfaced.
