13-193(a) Equipment must be ran to obtain one turnover every 30 minutes for a spa. Observed: flow meter read 55 GPM, required flow rate is 66.67 GPM

13-189(c)(4). Pool water shall be tested for disinfectant and pH at least 2 times each day. Observed:1 daily testing for the pool, evidence of fabrication of data as days listed with two readings had the exact same numbers and were recorded at the same time

13-202(a)(2) Pool entrances must be through self closing, self latching doors. Observed men's and women's entrance doors not self latching. COS. Latches on both doors was fixed.

13-190(a) The equipment shall be operated continuously on 24 hour basis to obtain the minimum required turnover rate (30 minutes). Observed flowmeter reading at 55 gpm. Should be 67 gpm.

13-189(C)(4). Pool water shall be tested for chlorine and pH at least 2 times each day. Observed 1 daily testing for pool on most days.

13-187(c) Copy of pool operator's certificate shall be displayed near the permit. COS. Certificate was posted.

13-201(c)(3) Bathroom sinks must have hot and cold water. Women's bathroom sinks did not have hot water. *Pool can remain open.

13-201(k) At all times an adequate supply of liquid soap shall be provided in the bathrooms. Observed 3 soap dispensers in women's bathroom without soap. Pool can still remain open because other dispensers had soap.

13-189(c)(1) DPD test kit or other approved method for testing chemicals shall be used. Observed expired test kit chemicals for test kit in the equipment room.

13-189(c)(5) All pools shall be kept free from sediment, dirt or debris and visible scum. Observed stains on pool plaster surface.

Observed lock on men's locker room detached from door. CA: 13-202(d)(3) When the pool is not open for use, access to the pool area shall be prohibited by the closing and securing of all entry.

Observed flow meter inoperable during inspection. Flow meter reading at 0gpm. CA: 13-190(a)(7) Flow meter must accurately indicate the circulation rate in gallons per minute. COS: Pool operator fixed flow meter on spa to read 100gpm.

Observed the same exact number written down for the chemical readings in the log book. CA: 13-189(c)(4) Pool chemicals must be tested daily and at least twice a day when pool is in use; pool chemical record logbooks indicate pool is being tested only once a day.

Observed test kit chemical testers with expired dates. Operator stated that the bottles are refilled. CA: 13-189(c)(1) Must have approved test kits.

Observed spa without permit. CA: 13-202(i) Permit shall be framed and displayed in conspicuous place.

Observed missing completion of drill in emergency action plan. CA: 13-187(c) "Drill procedures for emergency situations" must be written and provided in case of emergency.

13-202(a)(2) All pool entrances must self latch and self close. Observed men's and women's pool entrance unable to latch. The entrances were also unable to be locked.

13-202(h) The telephone number of the pool operator, manager, and of the Health Department shall be posted at the telephone. -Observed emergency phone operable, but required emergency numbers (health department and management) not posted next to emergency phone.

13-202(d) Lifesaving equipment shall be mounted in a conspicuous place and be readily available for use and in good condition. -Observed life ring tossed in a corner of the pool deck without a length of rope attached.

13-202(d) Additional sets of lifesaving equipment are required for pools longer than 50’ and shall be provided for each 50’ or pool length or major fraction thereof. -Observed only one set of safety equipment in the pool area.

13-205(d)(1) A clock, with a minimum 12-inch face shall be provided & mounted vertically so as to be clearly seen by all bathers in the whirlpool and whirlpool area. -Observed no clock mounted in the spa area visible to bathers.

13-190(a) Flowmeter must be operated continuously; observed flowmeter to be not functional.

13-188(a) Pool surfaces must be in good repair; pool surfaces observed to be heavily stained and pitted in areas. Please repair by next pool season.
