Observed food employee with artificial nails/nail color on while handing food/beverages. CA: Employees shall keep their fingernails clean and trimmed to no longer than the tips of the fingers. Unless wearing gloves in good repair, a food employee may not wear fingernail polish or artificial fingernails when working with exposed food. COS: Gloves was properly utilized while handling food.

Observed the entrance to the facility door open upon walking in for inspection. Observed gaps along the back door of the facility. CA: outer openings of a food service establishment shall be protected against the entry of insects and rodents by: Filling or closing holes and other gaps along floors, walls, and ceilings; Closed, tight-fitting windows; and Solid, self-closing, tight-fitting doors.

Observed exterior sides of flat grills and deep fryers with a heavy accumulation of grease and food particles, Observed the top of the reach in coolers to be accumulated with grease particles. CA: Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations .The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.

Observed the dumpster area with standing water that has dirt/ debris accumulation. CA: An outdoor storage surface for refuse, recyclables, and returnable shall be constructed of nonabsorbent material such as concrete or asphalt and shall be smooth, durable, and sloped enough to drain to prevent the collection of surface water.

Observed multiple florescent light bulbs missing shields in the walk in cooler. CA: Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food; clean equipment, utensils, and linens; or unwrapped single-service and single-use articles

Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed. Observed food worker chewing a wooden tooth pick while chopping onions for customer consumption in main kitchen. COS: Food worker removed tooth pick , washed hands and donned new gloves.

Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. Observed three containers of assorted RTE toppings for pancakes and waffles stored uncovered in four door RIC and four containers of assorted waffle and pancake batters stored uncovered in bottom area of prep top cooler. COS: PIC had food worker cover said food items.

Cleaning of Equipment and Utensils.(a) Equipment, Food-Contact Surfaces, and Utensils.1. Equipment food-contact surfaces and utensils shall be clean to sight and touch. Observed black like substance and an accumulation of condensate liquid on upper interior surface area of ice machine with ice for customer consumption. Additionally, multiple steak knives and ice scoop were observed stored in metal containers observed with black like substance COS: PIC had food worker discard said ice and wash, rinse and sanitizer equipment and utensils.

Ready-to-Eat, Time/Temperature Control for Safety Food, Disposition. A food that requires date marking shall be discarded if it exceeds 7 days. Observed individual containers of RTE diced tomatoes and sliced habanero peppers stored in 4 door RIC date marked to discard by 11/5/25. COS: PIC discarded said food items.

Jewelry. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. Observed food worker wearing three plastic bands while preparing chopped onions for customer consumption in main kitchen. COS: Food worker removed said bands , washed hands and donned new gloves.

Storage of In-Use Utensils. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: In a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at a frequency specified under DPH Rule 511-6-1-.05(7)(b)3(vi). Observed scoop for TCS whipped butter stored in a container of still water tested at 128F. COS: Utensil was removed and replaced with a clean and dry scoop.

Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations .The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Observed floors/ wall junctions behind food storage equipment with an accumulation of grease, standing water and old food particles in main food prep area in kitchen. PIC was advised to have area cleaned on a frequent schedule to avoid accumulation.

Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food; clean equipment, utensils, and linens; or unwrapped single-service and single-use articles. Observed multiple florescent light bulbs missing shields in the food prep, food storage area and dishwashing area through out the main kitchen. PIC was advised to have the shields replaced.

Observed the inside of the microwave to be unclean with black-like and greenish substances. CA: All food contact equipment shall be kept clean, Equipment used for storage of packaged or unpackaged food, such as a reach-in refrigerator, and the equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residue.

Observed dead roaches around the kitchen area. CA: Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.

Observed the floor tiles to be broken and the floors to be unclean with grease and debris behind equipment. CA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.

Observed interior of microwave unclean with debris in direct contact with food contact surface. COS- Microwave was cleaned and sanitized.

Observed cook wearing watch while preparing. COS- Cook removed watch washed and sanitized hands.
