Observed upon entry employee preparing food not wearing a hairnet or hats. COS: Employee donned hairnets. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.

Observed wiping cloths stored in solution with 0 chemical concentration. COS solution was remade to 200ppm quat.

Facility has unpaid permit fees.

Observed no paper towels at handwash station next to ice machine upon entry. COS PIC replaced paper towel.
