COS: PIC moved some bags of cooked beef into new container and stored in cooler below. Staff member placed bag of lettuce in cooler below prep-top. Temperature of bag of lettuce was rechecked in an hour and read as 44.

CA: Except during preparation, cooking, or cooling, or when time is used as the public health control, TCS food shall be maintained at 41°F.

Observed TCS (time/temperature controlled for safety) food cold holding at temperatures above 41F. Observed a container of pico de gallo (with diced tomatoes) cold holding at 51F, located on an ice bath. The PIC lifted the container and we observed that the ice level was too low and was not in contact with the pico de gallo container. Advised to ensure the ice bath is touching all side of the container. COS: PIC stated the container of pico de gallo had been out for less than 2 hours. PIC added ice, so the ice was in contact with all sides of the pico de gallo container.
