2-1B - hands clean and properly washed
Regulation: 511-6-1.03(5)(c) - when to wash (p)
Observed employee drop food item on the floor pick it up and take off gloves, but did not wash her hands before putting on new gloves to prepare a sandwich. Employee must wash their hands when potential cross contamination occurs and before putting on new gloves. Discussed with PIC -- staff members all washed their hands.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Build of mold observed at ice machine chute. Ice Machine nozzles/ chute need to be cleaned site to touch to avoid build up. Showed PIC who called second staff member over to start cleaning.

Regulation: 511-6-1.02(1)(d) - displaying of the inspection report (c)
Wrong inspection was being displayed at time of inspection. Need to display current inspection for public view, not historical inspection.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed Walk in freezer door jam busted, and water leaking on to floor under the walk in freezer evident by rust on floor leaking between WIC and Walk in freezer-- tiles were observed busted due to water damage and leak. Leaks need to be repaired and proper drainage needs to be installed and or fixed to WIC and WIF and floor tiles need to be fixed and replaced.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
There is broken tile near walk-in cooler. All physical facilities shall be maintained in good repair. CA: Replace broken tile.
