6 - premises free of pests and rodents; pesticide applied in accordance with laws, rules, and label
Regulation: 511-6-2-.13(1) - insect and rodent control
Observed dead/live pests such as roaches in lobby area and in rooms 127, 300, and 305. CA: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents. Restricted use pesticides, as specified in 40 CFR 152 Subpart I - Classification of Pesticides shall be applied only by a licensed professional. All pesticide applications must be in accordance with current state and federal laws and the product label. Applying pesticides within buildings using area fog dispersal methods or as warranted under DPH Rule-.20(4)(c) is restricted to licensed pest control professionals as part of an integrated pest management program. Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
Observed the facility with no annual gas fired equipment inspection on file. CA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency. PIC was provided with a copy of the annual gas fired inspection sheet and will contact a qualified licensed contractor or local fire authority to conduct an inspection
28 - beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered
Regulation: 511-6-2-.14(6) - bed, bedding, and linens
Observed stained bedding in room 100. CA: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly. PIC will have the stained linen replaced.
Observed the beds in rooms 301, 303, 311, 315, 325, 327, and 329 with no sheets, pillows, and pillowcases. CA: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Observed no copy of the most recent inspection report being displayed for public view. CA: The most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away. PIC will display the inspection report in public view

Observed no toilet tissue provided at the toilet in rooms 100, 301, and 305. CA: Toilet tissue shall be provided in a dispenser at each toilet at all times. PIC will provide toilet tissue in each of the rooms
24 - openings to outside and physical facilities protected by tight-fitting doors, screening, or other means
Regulation: 511-6-2-.13(3) - insect and rodent control-openings
Observed 3rd floor hallway air conditioning unit completely detached from the wall. CA:Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means. Screen doors shall be self-closing and screens for windows, doors, skylights, transoms and other openings to the outside shall be tight-fitting and free of breaks. Screening materials shall not be less than sixteen mesh to the inch. Screen doors for sliding patio doors will not be required to be self closing except in food service, preparation and utensil washing areas of permitted food service establishment kitchens and bed and breakfast inns. PIC will have unit repaired
19 - anti-slip tubs/showers, slip strips, mats or appliqué products provided and in good condition

Observed no anti slip mats in the tubs in rooms 100, 301, and 305. CA: Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair. PIC will provide the tubs in the rooms with an anti- slip mat
