Observed handwashing sink without paper towels. CA: Food employees shall clean their hands in a handwashing sink or approved automatic handwashing facility and may not clean their hands in a sink used for food preparation or warewashing, or in a service sink or curbed cleaning facility used for the disposal of mop water and similar liquid waste. PIC provided paper towels for handwashing sink.

Observed pork on counter and raw chicken in walk in cooler stored uncovered. CA: Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. PIC covered food items.

Observed shrimp being thawed without being held under running water. CA: Except as specified in paragraph 4 of this subsection, time/temperature control for safety food shall be thawed: Under refrigeration that maintains the food temperature at 41°F (5°C) or less; PF or Completely submerged under running water: At a water temperature of 70°F (21°C) or below. PIC put shrimp under running cool water.

Observed cardboard being used as mats in walk in cooler. CA: Nonfood-contact surfaces shall be free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance. PIC removed cardboard from ground.
15C - nonfood-contact surfaces clean

Observed fume hood in need of cleaning. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. PIC will have fume hood cleaned.
