Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
Observed no means of verifying employees were informed of their responsibility to report information about their health as it relates to food borne illnesses and symptoms. CA: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. PIC was given a health reporting agreement and will have food employees read and sign.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(b) - hand drying provision (pf)
Observed handwashing sink without a supply of paper towels for drying hands. CA: Each handwashing sink or group of adjacent handwashing sinks shall be provided with:
- Individual, disposable towels; Pf
- A continuous towel system that supplies the user with a clean towel; Pf
- A heated-air hand drying device; Pf or
- A hand drying device that employs an air-knife system that delivers high velocity, pressurized air at ambient temperatures.Pf PIC provided paper towels for handwashing sink.
10D - food properly labeled; original container

Observed no allergy disclosure for consumers in the facility. CA: The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. PIC will provide allergy disclosure statement to consumers.
14C - single-use/single-service articles: properly stored, used
Regulation: 511-6-1.05(10)(e)1&3 - single-service/single-use items stored 6" off floor in clean, dry location (c)
Observed a box of single use forks stored uncovered on the counter. CA: Single-service and single-use articles shall be stored as specified under paragraph 1 of this subsection and shall be kept in the original protective package or stored by using other means that afford protection from contamination until used. PIC closed the box of forks.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(d) - nonfood-contact surfaces (c)
Observed an accumulation of grease on fume hood. CA: Nonfood-Contact Surfaces. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. PIC will have fume hood cleaned.
