Observed most recent inspection report not posted in facility. CA: The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. PIC will have most recent inspection report posted.

Observed food service permit not posted in the facility. CA: Post the permit in a location in the food service establishment that is conspicuous to consumers. PIC will have food service permit posted.

Observed dead pests such as roaches throughout the base of operation. CA: Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests. PIC will have the dead pests cleaned up.

Observed the interior of the RIC with an accumulation of food residue. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. PIC will have the interior of the RIC cleaned.

CA: The premises shall be free of items that are unnecessary to the operation or maintenance of the establishment, such as litter or equipment that is nonfunctional or no longer used and litter. PIC will have the items removed.

Observed rodent in the base of operation and the presence of rodents need to be minimized by routinely inspecting the premises for evidence of rodents. CA: 511-6-1.07(5)(l) - Removing Dead or Trapped Birds, Insects, Rodents, and Other Pest (C) (l) Removing Dead or Trapped Birds, Insects, Rodents, and Other Pests. Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests. PIC will clean up the rodents.

Observed no hot water at the hand sink in the restroom. CA: 511-6-1.06(1)(g),(h) - Water Supply, Capacity; Pressure (Pf) (g) Capacity. 2. Hot water generation and distribution systems shall be sufficient to meet the peak hot water demands throughout the food service establishment. PIC will have the hot water issue corrected.

Observed the interior of the RIC not free from an accumulation of food residue. CA: 511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. PIC will clean the interior of the RIC.

Observed the walls in the restroom are not in good repair (paint peeling). CA: 511-6-1.07(5)(a),(b) - Good Repair, physical facilities maintained; Cleaning, Frequency & Restrictions, cleaned often enough to keep them clean (C) (a) Good Repair. All physical facilities shall be maintained in good repair. PIC will have the walls painted.
