Observed no employee health agreements or satisfactory demonstration that employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. CA: PIC must provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food. Must be corrected 10 days.

Observed empty bottle of EPA registered norovirus cleaner on site. CA: Facility shall maintain access to an EPA registered disinfectant for vomit and fecal clean up. Must be corrected in 10 days.
