Observed brown residue seen on white panel inside ice machine and rust soil buildup inside ice machine on lid - ice machine deterioration causing rust on edges of the bin. Observed slight brown residue on lid of ice bin in bar area. Observed black residue on can opener blade on shelf in prep area next to bar. Observed black stains embedded on 2 cutting boards on dish drying shelf in prep area next to bar CA: Food contact surfaces shall be clean to sight and touch. COS:PIC cleaned listed equipment

Observed pizza dough being prepped and plastic dough bins stacked on top of an open trash can in prep area next to bar. CA: Cross contamination from a dirty surface to clean equipment must be prevented COS:PIC removed dough bins

Observed produce prep sink being used to thaw raw chicken. CA: Produce prep sink must only be used for the purpose of thawing fresh fruits and vegetables. COS: Employee removed chicken

Observed double doors in dining area with gaps creating openings between the doors causing sunlight to show through gaps. CA: Facility must be in good repair- equipped with tight fitting building components to enclose facility and help prevent pest access
