Establishment does not have established procedures for employees to follow when responding to vomiting or diarrheal events. CA: Establishment must have procedures for employees to follow when responding to vomit or diarrheal events. Must be corrected in 10 days.

Observed the following foods stored uncovered: beef in walk in cooler; shrimp in prep top cooler of main kitchen; chicken broth, rice and salsa in reach in cooler. CA: Foods shall be stored covered. COS: PIC covered all mentioned foods and discarded rice and salsa

Observed an accumulation of excess black residue on soda dispensing nozzles of soda machine in hostest area and inside ice bin on lid in bar. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: Employee cleaned ice machine and ice bin

Observed old food stuck to food contact surface of metal containers and metal lids stored on shelf under prep table; skillet, bowls, plates, scoops and utensils on air drying rack; cutting board on prep top cooler in main kitchen. CA: Food contact surfaces shall be stored cleaned to sight and touch. COS: PIC moved dishes to dirty dish area to be recleaned

Observed several boxes and containers of food and fresh vegetables stored on floors of walk in cooler in main kitchen. CA: Food shall be stored with a clearance of 6 inches from floor

Observed prep table in main kitchen with a mix of customer foods, single use utensils, kids shoe, medicine bottles, excess papers, tools, electronics, personal bags, phone, speaker. Observed employee food and food for customer stored together in reach in cooler of main kitchen. CA: Employee items must be stored separate from customer foods and kitchen equipment.

Observed vegetable sink stacked with dirty dishes and with end of mop sink hose stored inside. CA: Vegetable sink shall be used only for the purpose of washing fruits and vegetables.

Observed facility utilizing cardboard to line the floors under the grill and as lining for shelves holding food and equipment. CA: Nonfood contact surfaces of equipment must be constructed of corrosion resistant, nonabsorbent, and smooth materials.

Observed excess black residue on walls of mop sink area, food residue on floors behind cooling equipment, spider webs in ceiling corners of walk in cooler and dishwashing area, and accumulated residue on dry storage shelves, rust residue on shelving under prep tables in main kitchen. Nonfood contact surfaces shall be maintained clean.

Observed walk in cooler used to store excess with harborage of plastic crates, boxes, containers, coolers and other equipment. CA: Facility must have adequate capacity to store refuse, trash, or unused equipment.

Observed ice machine ceiling in main kitchen in disrepair: board used as a ceiling to cover ice - gaps and openings between machine and board. CA: Equipment and utensils must be properly designed and constructed and maintained in good repair.

Observed back door frame and part of wall constructed of excess soft foam caulk of kitchen leading to the outside walk-in cooler. Observed back door of dish washing area with gasket in disrepair - sunlight seen under bottom of closed door and gap in ceiling tile in dish washing room. CA: All nonfood contact equipment must be designed and constructed in durable manner and maintained in good repair.

Observed several pots, skillets and pans stored for use on dish rack with food contact surfaces with signs of visible wear, surface stains, corroded surfaces. CA: Utensils and/or food contact surfaces of equipment must be made of materials that are safe, durable, corrosion-resistant, and nonabsorbent.
