OBSERVED MULITPLE FOOD BEING STORED UNCOVERED IN THE MAIN KITCHEN PREP TOP COOLER-BOTTOM #1 (I.E. LIQUID EGGS, RAW SHRIMP, PORK, LO MEIN NOODLES, AND CUT CABBAGE). CA: Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination; COS: PIC COVERED ITEMS.
4-2B - food-contact surfaces: cleaned & sanitized

OBSERVED SEVERAL STAINLESS-STEEL CONTAINERS, KNIVES, AND A CAN OPENER STORED AS CLEAN IN THE MAIN KITCHEN WAREWASHING AREA. OBSERVED BLACK RESIDUE ON THE INTERIOR PORTION OF THE ICE MACHIME. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. Pf. COS: PIC REMOVED ITEMS FOR PROPER CLEANING.
6-2 - proper date marking and disposition

OBSERVED NO DATE MARKING FOR 2 CONTAINERS OF PORK IN PREP TOP-BOTTOM #1, 1 CONTAINER OF LO MEIN IN PREP TOP-BOTTOM #1, 2 CONTAINERS OF LO MEIN IN THE WALK IN COOLER, AND 1 CONTAINER OF RAW CHICKEN IN THE WALK IN COOLER THAT WERE BEING HELD OVER 24HRS. CA: time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days. OBSERVED PIC WASH FOOD GRADE PLASTIC CONTAINER AND RINSE HANDS IN THE VEGETABLE SINK. CA: THE VEGETABLE SINK CAN ONLY BE USED FOR WASHING FRUITS AND VEGETABLES. COS: PIC WASHED CONTAINER IN THE 3 COMP SINK AND WASHED HANDS AT HAND SINK.
18 - insects, rodents, and animals not present

OBSERVED BACK DOOR AND MESH SCREEN OPEN TO THE OUTSIDE ALLOWING SMALL PESTS ACCESS TO THE MAIN KITCHEN. CA: Outer Openings, Protected.
- Except as specified in paragraphs 2 through 5 of this subsection, outer openings of a food service establishment shall be protected against the entry of insects and rodents by:
(i) Filling or closing holes and other gaps along floors, walls, and ceilings;
(ii) Closed, tight-fitting windows; and
(iii) Solid, self-closing, tight-fitting doors.

The requirements in paragraph 1 of this subsection does not apply if a food service establishment opens into a larger structure, such as a mall, airport, or office building, or into an attached structure, such as a porch, and the outer openings from the larger or attached structure are protected against the entry of insects and rodents.

Exterior doors used as exits need not be self-closing if they are:
(i) Solid and tight-fitting;
(ii) Designated for use only when an emergency exists, by the fire protection authority that has jurisdiction over the food establishment; and
(iii) Limited-use so they are not used for entrance or exit from the building for purposes other than the designated emergency exit use.
- Except as specified in paragraphs 2 and 5 of this subsection, if the windows or doors of a food service establishment, or of a larger structure within which a food service establishment is located, are kept open for ventilation or other purposes or a temporary food service establishment is not provided with windows and doors as specified under paragraph 1 of this subsection, the openings shall be protected against the entry of insects and rodents by:
(i) 16 mesh to 1 inch (16 mesh to 25.4 mm) screens;
(ii) Properly designed and installed air curtains to control flying insects; or
(iii) Other effective means.
- The requirement in paragraph 4 of this subsection does not apply if flying insects and other pests are absent due to the location of the establishment, the weather, or other limiting condition.
16B - plumbing installed; proper backflow devices

OBSERVED CARBOARD BOX CONTAINING A PLASTIC CONTAINER WITH STAINLESS STEEL FILTER CATCHING FOOD DEBRIS UNDERNEATH VEGETABLE SINK IN THE MAIN KITCHEN. OBSERVED PLASTIC CONTAINER WITH A STAINLESS-STEEL FILTER BEING HELD UP BY ROCKS UNDERNEATH THE 3 COMP SINK CATCHING FOOD DEBRIS IN THE MAIN KITCHEN. CA: System Maintained in Good Repair. A plumbing system shall be repaired according to law; P and maintained in good repair.
14B - utensils, equipment and linens: properly stored, dried, handled

OBSERVED NUMEROUS STAINLESS-STEEL CONTAINERS STACKED WET IN THE MAIN KITCHEN WAREWASHING AREA. CA: Equipment and Utensils, Air-Drying Required. After cleaning and sanitizing, equipment and utensils:

Shall be air-dried or used after adequate draining before contact with food; and
-
May not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry.
