Facility unable to provide proof of an annual gas fire heating equipment inspection report within the last year. Facility presented inspection report from year 2024. CA: Facility must ensure a gas fire heating equipment inspection is completed annually and documents kept on file. Must be completed by July 1, 2026

Observed ice machine on 2nd floor with build up of black and brown residue on inside of ice dispenser spout. CA: Food contact surfaces shall be clean to sight and touch. Must be corrected by March 30th

Observed a spray bottle with pink liquid on housekeeping cart located on the 1st floor. Observed cup with lid and straw stored on a linens shelf above clean folded towels in housekeeping laundry room on the 1st floor. CA: Housekeeping chemical bottles shall be labeled with its contents. Employee drinks shall be kept separate from materials for guests to avoid cross contamination. Housekeeper removed employee drink to a break room area
