6-1A - proper cold holding temperatures

Observed raw shrimp contained in beer cooler stored at 52F.
CA: Employee discarded shrimp immediately.
6-1B - proper hot holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; hot holding (p)
Observed stove holding vegetable/meat soup at 120F.
RCA: All items in hot holding must be held at 135F or higher.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed prep counter used to prep vegetables and meats/seafood to be covered in yellow debris and heavy buildup. Observed grill to have excessive grease and food debris stored on the food contact-surface. Observed ice scoops and sugar scoops stored with the handle touching the food product.
RCA: Clean equipment regularly to eliminate contamination. Ensure handles of scoops are placed in containers with the handle facing up to eliminate contamination.

Observed raw chicken wings being thawed at room temperature.
CA: Employee turned water on to run over chicken and was informed of process to thaw raw meats.
16A - hot and cold water available; adequate pressure
Regulation: 511-6-1.06(1)(g),(h) - water supply, capacity; pressure (pf)
Observed hot water in public restrooms reading at 60F. Observed water pressure to not be functioning properly within the hot water section of the water heater.
RCA: Repair water heater supplying water to public hand sinks in both male and female restrooms. Ensure adequate pressure is available.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(2)(a) - equipment and utensils, constructed of durable materials (c)
Observed rags being used as shelf liner to store food contact (plates) and other non-food contact items.
RCA: Use commercial grade shelf liner to store items on shelves.
15B - warewashing facilities: installed, maintained, used; test strips
Observed expired test strips and quaternary tablets.
RCA: Buy new tablets and test strips to test sanitizer buckets.
17A - toilet facilities: properly constructed, supplied, cleaned
Regulation: 511-6-1.06(5)(h) - toilet room receptacle, covered (c)
Observed employee restroom missing lidded trash receptacle.
RCA: Place lidded trash can in employee restroom for sanitary napkins.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed floors, walls, and ceiling tiles coated with excessive debris, rust like material, or dust. Observed excessive amounts of food debris on floor surrounding prep counter.
RCA: Clean floors, walls, and ceiling tiles regularly to eliminate chance of contamination by debris.
