Observed cheese in a metal container in the stand-up reach in cooler stored without a lid.
CA: PIC placed a lid on the container.
10D - food properly labeled; original container
Regulation: 511-6-1.04(4)(d) - food storage containers identified with common name of food (c)
Observed plastic bottles in facility not labeled with sauce name. Fourth repeat violation. Due to staff change, PIC was informed that another violation may result in an informal hearing.
RCA: PIC is to ensure proper labeling of sauces or follow a color coordinated system if labels fall off from washing bottles.
11D - thermometers provided and accurate
Regulation: 511-6-1.05(2)(x) - tmd, located in refrigerators/ hot holding (pf, c)
Observed multiple cold holding coolers missing internal hanging thermometers.
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(e)1,2,4 - equipment, utensil, linens, stored 6" off floor in clean, dry location (c)
Observed food containers used in cabinet warmers stored right side up and not inverted.
RCA: Ensure all equipment used to serve or prepare food is inverted after being clean and sanitized to prevent accumulation of dust, debris, or other contamination sources.
14C - single-use/single-service articles: properly stored, used
Regulation: 511-6-1.05(10)(e)1&3 - single-service/single-use items stored 6" off floor in clean, dry location (c)
Observed cups stored in ceiling where ceiling tile is missing in main kitchen.
RCA: PIC will throw cups away and repair ceiling tile.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed accumulation and buildup of food debris and grease on freezer near the fryer, the walls, and the underside of the oven. Observed splattering on wall near coke machine in main dining area.
RCA: Commit to a thorough cleaning schedule every day and after each rush to ensure the cleanliness of equipment and non-food contact surfaces. Rule: 511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.
- Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed floors, walls, and behind equipment holding debris and food splatter. The vents in the main dining area have dust and rust debris accumulating.
RCA: Increase cleaning frequency.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed two drink cups with lid and straw stored near food preparation counters.
CA: PIC removed cups and threw them away.
