Observed working containers at grill and bulk food container without "common" named marked on label. Labels being added to bulk + working containers. COS.

Regulation: 511-6-1.06(2)(j) - backflow prevention device when required (p)
Observed mop sink hose attached to faucet without proper vacuum breaker. Add vacuum breaker to cold faucet, place hose on hot faucet (has vacuum breaker), or relocate hot line breaker to cold line. Hose disconnected.

Observed drink station shelf and chopping block shelf in rough/not smooth and/or easily cleanable condition. Resurface or repair so as to provide a smooth, non-absorbent, durable and easily cleanable surface, or replace shelves. Complete by 2/21/26

Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed accumulations of grease, oils, food particles, and/or debris on hood filters, ansul system, walls by cook line, floors under cook line, and walks and floors at mop sink. Clean surfaces at a frequency to prevent accumulations. Correct by 2/21/26.
