Regulation: 511-6-1.05(10)(g)(1)&(3) - kitchenware and tableware, single-service/-use items properly handled to protect food-/lip-contact surfaces (c)
Observed self-serve coffee + to go cups stored in open/uncovered holders exposing them to potential sources of contamination. Keep cups in covered dispensers, or leave in protective plastic sleeves, with only the bottom of the top cup exposed. COS.
17C - physical facilities installed, maintained, and clean

Observed damaged floor covering in area of upright 2 door freezer (in hall) and accumulations of scale, debris under upright reach in freezer(kitchen). Repair surfaces/flooring to provide a smooth and easily cleanable surface, and clean floors at a frequency to prevent accumulations. Correct by 2/21/26
