Observed employees going between tasks such as handling a cell phone and then going back to prep food without washing hands. Employee was retrained to wash hands and washed hands before returning food prep. Employees shall change wash hands whenever engaging in an activity that could contaminate the hands.

Observed deep covered pans of rice, beans, and cooked meats being cooled in ambient temperature without ice, or storing. Foods that were above 135F-70F had been out for less than 2 hours and were placed into a freezer to rapidly chill.
