Observed employee drinks in non-single-use containers without lid and straws throughout the kitchen; observed reusable cup with a lid but no straw on the front line, an energy drink can on the line, and then several reusable drinks with lids but no straws on the back dry storage rack. COS. Removed. Code: Employee drinks shall be stored in single-use containers with a lid and a straw and shall be handled to prevent contamination.

Observed that the facility has self-metering/self-closing faucets in the restrooms that shut off after 3-5 seconds while trying to wash hands. Code: Hand sinks with self-closing/self-metering faucets shall run for at least 15 seconds without needing to be reactivated.

Observed that the facility does not have all required parts of the consumer advisory on the menu; facility has the identifying asterisk and the reminder statement, but the disclosure is missing; observed that the consumer advisory on the menu board is smaller than the other smallest print on the menu. Code: If a raw animal product such as meats or eggs may be served undercooked or raw, a consumer advisory shall be present and include all required portions including the disclosure, the identifying asterisk, and the reminder statement.

Observed wet wiping cloths stored on counters at prep station. COS. Wet wiping cloths placed into sanitizer solution. CA: Wet wiping cloths shall be stored in sanitizer solution at the appropriate concentration between uses.

Observed that the sealed concrete in the front food service area has started to wear down and is no longer properly sealed. Recommended action: Reseal floors. Code: Floors, walls, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.

COS. Moved. Code: Employee belongings and activities shall be in designated locations that prevent contamination of food and food contact surfaces.
