Observed that the facility was using the dishmachine and that no sanitizer was being dispensed. COS. Dishes shall be hand washed, rinsed, and sanitized until the dishmachine is serviced. CA: Mechanical warewashing shall dispense the sanitizer at the correct concentration.
Regulation: 511-6-1-.04(7)(e) - consumer advisory provided for raw/undercooked
Observed that the menu did not have a consumer advisory. CA: Facilities that serve raw or under-cooked foods shall have a consumer advisory on the menu that states one of the two following: (1) Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness (2)Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness, especially if you have certain medical conditions. Consumer advisory shall be at least 8 point font and each item shall be asterisked as well as the consumer advisory. There shall be a statement stating what items are raw or that shall be included in the item description.

Observed that some items were date marked only with the day of discard while others had the day of preparation and observed items that were date marked for 8 and 9 days instead of 7. COS. Dates fixed. CA: Prepared TCS items shall be date marked to be held for no longer than 7 days with the day of preparation being day 1; shall be marked with the date of preparation, the date of discard, or both, however, the method of date marking shall remain consistent.

Observed employees preparing food while wearing jewelry. COS. Removed. CA: With the exception of a plain band ring, no jewelry may be worn on the hands or wrists while preparing food to include medical information bracelets.

Obsevred that the facility was thawing raw meats in the vegetable sink. COS. Meats were moved to the meat sink and vegetable sink was washed, rinsed, and sanitized. CA: The produce sink shall only be used for washing produce and thawing/preparing RTE food.

Observed that the inspection report posted was not the most recent inspection report and that the place where the inspection report was posted was behind chairs and items making unable to be read from a distance of 1 foot away. CA: The most recent inspection report shall be posted within 15 feet of the front door, 5 to 7 feet from the floor, readable from a distance of 1 foot away, and in a conspicuous place.
