Observed that the squirt bottles on the line were not labeled. COS. Labels added. Code: Non-easily identifiable foods shall be labeled with the common name of the product.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(g) - jewelry (c)
Observed employee with large, detailed ring on their finger while preparing food (drinks). COS. Removed. Code: With the exception of a plain band ring, no jewelry may be worn on the hands or wrists while preparing food.

Observed employees preparing food (drinks) while having fingernails longer than the tips of their fingers. COS. Hands washed and gloves donned. Code: Fingernails shall be kept free of paint or polish and trimmed unless gloves are worn while handling food.
12C - wiping cloths: properly used and stored
Regulation: 511-6-1.04(4)(m) - wiping cloths, use limitation (c)
Observed wet wiping cloth stored on the counter on the line. COS. Placed into sanitizer solution. Code: Wet wiping cloths shall be stored in sanitizer solution at the appropriate concentration between uses.

Observed that the door sweep has worn away causing there to be a large gap under the rear exterior door. Code: Exterior doors shall be self-closing and tight-fitting or otherwise protected against pest entry.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(2)(a) - floor, walls, & ceilings, cleanability (c)
Observed that the floor at the dish pit has started to be worn down with the concrete no longer being sealed and have many divots/holes in it; concrete floor is not easily cleanable. Code: Floors, walls, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed personal belongings stored over the back prep counter in between the prep sinks, on the counter at the bar/on top of the reach-in keg cooler, and stored with/above the single-use items in the upstairs storage area. COS. Removed. Code: Employee belongings and activities shall be in designated locations that prevent contamination of food and food contact surfaces.
