Observed that the food service permit was not posted. CA: The food service permit shall be posted within 60 days.
14A - in-use utensils: properly stored
Regulation: 511-6-1.04(4)(k) - in-use utensils, between-use storage (c)
Observed that bowls were being used as scoops and the bowls are stored in the product. CA: In-use utensils shall have handles and those handles shall be stored above the level of the food if stored in the food.
14C - single-use/single-service articles: properly stored, used
Regulation: 511-6-1.05(10)(e)1&3 - single-service/single-use items stored 6" off floor in clean, dry location (c)
Observed single-use items stored upright without overhead protection. COS. Items were inverted. CA: Single-use containers shall be stored upright with overhead protection or shall be stored inverted.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed accumulation of debris in/on units. CA: Equipment shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed accumulation of debris on the floors throughout the facility, particularly under shelves and equipment and in walk-in's. CA: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee belongings stored with and above food and food contact surfaces. COS. Removed. CA: Employee belongings and activities shall be in designated locations that prevent contamination of food and food contact surfaces.
