Observed food items in the warmer cabinet holding temperatures below 135 degrees F.(see temperatures above) COS reheated. Code: TCS food that uses hot holding as a control for safety shall be held at 135 F or higher.
2-2A - management knowledge, responsibilities, reporting
Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
Observed that the facility did not have verifiable evidence of employee health reporting acknowledgement forms. Code: Facility shall have verifiable means of showing employee health reporting acknowledgement.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed employee hand washing sink in the main kitchen area was being blocked by two brooms and a dust pail. COS. items removed Code: A handwashing sink shall be accessible at all times for employee use.

Observed that the food service permit was not displayed. Code: The food service permit shall be displayed within public view.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(g) - absorbent materials on floors, use limitation (c)
Observed that the facility is placing cardboard on the floor; person-in-charge states they are this is to be used as a non-slip surface. COS. Discarded. Code: Absorbent materials shall not be used on floors.
