Observed that the facility could not provide signed employee health agreement forms. COS employee health agreement forms were signed. Code: Food employees and conditional employees shall be informed in a verifiable manner of their responsibility to report to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.07(3)(b) - hand drying provision (pf)
Observed handwashing sink with out paper towels. COS PIC refilled paper towel holder. Code: Each handwashing sink or group of adjacent handwashing sinks shall be provided with hand drying provisions.

Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed an accumulation of debris on the bottom of the RIC. Code: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
