2-1B - hands clean and properly washed
Regulation: 511-6-1.03(5)(c) - when to wash (p)
An employee walked out the back door of the facility, came back in, walked through the kitchen into the lobby, and then back into the kitchen, where they immediately began prepping pizza dough without washing their hands. COS manager educated staff on when to wash their hands. Code: Food employees shall wash their hands prior to working with food, when entering the kitchen, and after engaging in other activities that contaminate the hands.

Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed the walk-in freezer unit leaking resulting in pile of ice water on the floor. Code: Equipment shall be maintained in a state of repair and proper adjustment.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed accumulation of debris on the floor, especially in the walk in's. Code: Facility shall be thoroughly cleaned and cleaning frequency shall be enough to prevent accumulation.
