Observed employee drink stored in a reuseable tumbler cup with no straw. COS. Removed. CA: Employee drinks shall be stored in single-use containers with a lid and a straw and shall be handled to prevent contamination of food and food contact surfaces.
2-2D - adequate handwashing facilities supplied & accessible
Regulation: 511-6-1.06(2)(o) - using a handwashing sink- operation & maintenance (pf)
Observed that the handsink was blocked by a fan in the basin and the rice warmers being placed in front of it. COS. Sink cleared. CA: Hand sinks shall be kept clear and accessible at all times.
18 - insects, rodents, and animals not present

Observed door and window open without pest prevention. COS. Door closed, screen placed down. CA: Exterior openings shall be protected from pest entry.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.02(1)(f)(1) - permit, displayed (c)
Base violation: Observed that the permit was not posted. CA: The food service permit shall be displayed in public view.
15B - warewashing facilities: installed, maintained, used; test strips
Regulation: 511-6-1.05(3)(h),(i),(j) - temperature measuring device, manual warewashing; sanitizing solutions, testing device (pf)
Base violation: Observed that the facility does not have test strips for the QUAT sanitizer that is used. CA: Facility shall have the means to test each type of sanitization method used in the facility.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(2)(a) - floor, walls, & ceilings, cleanability (c)
Base violation: Observed that the sealant on the floor has started to wear down and there is now exposed, unsealed concrete. CA: Floors shall be easily cleanable.
