Observed that the facility washed their utensils and pots, but did not sanitize them before putting them onto the shelf and before using them again. COS. Sanitizer solution was made and dishes were sanitized. Code: Utensils and food-contact surfaces of equipment shall be sanitized before use after cleaning
8-2B - toxic substances properly identified, stored, used
Regulation: 511-6-1.07(6)(o) - medicines, restriction & storage (p, pf)
Observed that the facility was keeping ibuprofen, Hydralazine, and vitamin C with and above food contact surfaces on the line. COS. Medications were removed. Code: Medicines that are in a food service establishment for the employee’s use shall be labeled with a legible manufacturer’s label and located to prevent the contamination of food, equipment, utensils, linens, and single-service and single-use articles; and located so they are inaccessible to children.

Observed that the facility is keeping cleaners and hand soap with and above food (sauces, dry goods) in the storage area. COS. Removed. Code: Chemicals shall be stored in a location and in a manner that prevents contamination of food and food contact surfaces.

Regulation: 511-6-1.05(1)(i) - nonfood-contact surfaces (c)
Observed that the facility is using cardboard as shelf liners/on top of equipment. Code: Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Observed that the floor in the main kitchen area has gashes in the expoy seal exposing unsealed concrete; observed that the ceiling tiles over the mop sink in the main kitchen area are porous drop-in tiles. Code: Floors, walls, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.

Observed that the facility lining portions of the floor on the line with cardboard; cardboard is heavily soiled. Code: Absorbent materials shall not be used on floors.
