Observed personal cup with no lid or straw stored on prep table in kitchen. CA: personal drinks must be in single use cup with lid and straw and stored in designated area away from food prep tables and food. COS: pic discarded personal drink.
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
Observed multiple bags of food stored unsealed/uncovered in reach in freezer chest and in walk in freezer. CA: Food should be stored covered or sealed properly. COS: pic covered food and discarded food with ice buildup on the food. (sausage/hotdogs)
6-2 - proper date marking and disposition
Regulation: 511-6-1.04(6)(g) - ready-to-eat time/temperature control for safety food, date marking (pf)
Observed TCS food (cheese), ranch, and food cooked and frozen with not date mark. CA: TCS food must be date marked and held for no more than 7 days, including commercially prepared food removed from original containers, deli meat and soft cheese once the seal is broken and food cooked and frozen. COS: pic discarded food.
12C - wiping cloths: properly used and stored

Observed wiping cloth sanitizer not at minimum ppm for wiping tables in dining room. CA: Chlorine sanitizer must be at the required ppm for sanitizing food contact surfaces including tables in dining room. COS: PIC remade sanitizer and tested for the minimum ppm for chlorine
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(q)1&3 - good repair & calibration (c)
Chest cooler not working/operating in kitchen. CA: Equipment must be maintained in good repair and operating properly. COS: pic will have chest freezer removed within 48 hours.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed outside of food storage containers with food build up including sauce bottles and the lids/tops of sauce bottles and ice build up on inside of freezer. CA: Nonfood contact surfaces must be cleaned to sight and touch. COS: pic will clean all surfaces including chest freezer. Health Dept will re-check
