Regulation: 511-6-2-.15(5) - heating and fire safety-inspection
PIC could not produce current Gas-Fired Equipment Annual Inspection Reports for any gas-fired equipment at facility. Advise PIC to have annual inspections of Gas-Fired Equipment performed and retain the records at the facility to show during inspections. POC in comment/remarks section of report

Regulation: 511-6-2-.13(2) - insect and rodent control-records
Inspector observed pest control records lacking 7 of the past 18 months required, not including the current month. Corrective action: A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises, except for the occasional use of consumer insect sprays on small spots in accordance with the product label. The record may be made by the permit holder or provided by the applicator, and shall list the following information: (a) amount and concentration of product used; (b) name of product used; (c) date and location of application; (d) application method; (e) pest targeted; and (f) name of applicator

Inspector observed mold-like growth on the FCS of common-use ice machines on floors 2 and 5. Corrective action: Ice machines shall be maintained in good repair, and cleaned and sanitized according to the manufacturer’s recommendations or at a frequency to preclude the accumulations of mold and soil
19 - anti-slip tubs/showers, slip strips, mats or appliqué products provided and in good condition
Regulation: 511-6-2-.08(10) - anti-slip surface
Inspector observed no anti-slip surface in the shower floors of ADA rooms 614 and 504. Corrective action: Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair
