Observed disfigured headboard in room 2001, frayed carpet in room 1925, disfigured lampshade disfigured in room 1921, a chair with a hole on side in room 2806. CA: Advised PIC to keep all furniture maintained and in good repair

Observed numerous stains or hair on pillow cases, mattress covers, or sheets in rooms 1934, 1130, 832, 803, 630, 3528, 3316, 2727 CA: Informed PIC that all pillows, pillow cases, mattress covers, and bedding shall be clean and free from stains Advised PIC to replace stained bedding

Observed broken window panel in hallway of the 33rd floor CA: Advised PIC to immediately repair window panel and informed that all openings to outside and physical facilities shall be properly maintained and in good repair

Observed numerous stains on curtains in rooms 1126, 1130, and 1036 as well as champagne stains on ceiling in room 2814. CA: Advised PIC to check and clean curtains regularly Observed broken window panel in hallway of the 33rd floor CA: Advised PIC to fix window panel and informed that floors, walls m ceilings, windows, and door should be properly maintained and in good repair

Observed pink and black build up in ice machines on floors 19, 32, and 33. CA: Advised PIC to regularly clean and monitor ice machines

Observed the facility has record of expired annual gas-fired equipment inspections on boilers and heaters. Corrective action - Engineering Director advised that inspections on all gas-fired equipment must be scheduled as soon as possible. All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed stains or hair on mattress covers, sheets, or pillow cases in rooms 3801, 3703, 3401, 3330, 3201, 2930, 2621, 502, 1118, 1520. Corrective action - PIC advised to review cleaning protocols with housekeeping staff. Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed stains on walls and floors; soiled dishware or food trays; or bags, boxes, and other items of trash on floors of landing storage areas and housekeeping rooms on floors 35, 36, 33, 29, 26, 38, 20.
Observed hole in wall on floor 36. Corrective action - PIC advised to provide trash cans for landing storage and housekeeping rooms as well as racks to place soiled dishware and food trays on prior to delivery to warewashing areas. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed excess dust, debris, hair, and trash items (bottle caps, paper scraps, metal items, coins, food items, and crumbs) on carpeted floors in rooms 634, 727, 829, 905, 1018, 1227, 1439, 1520, 1618, 1717, 1817, 1936, and 2036. Corrective action - PIC advised to review cleaning protocols with housekeeping staff and ensure vacuuming on floors under all furniture. Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed pink substance on chute inside ice machines on floors 38, 37, 36, 35, 34, 33. Corrective action - PIC advised that he is having someone clean all ice machines today, but will create a weekly project to ensure cleaning on ice machines going forward. Scoops, ice buckets, and other ice handling equipment, shall be of easily cleanable material and construction. They shall be stored in a clean place and shall be kept clean
