CA: Install an approved mop sink with a backflow device.
511-6-1.06(2)(i) - Service Sinks, Numbers and Capacities (C) (i) Service Sinks, Numbers and Capacities. At least one service sink or one curbed cleaning facility equipped with a floor drain shall be provided and conveniently located for the cleaning of mops or similar wet floor cleaning tools and for the disposal of mop water and similar liquid waste. Toilets and urinals may not be used as a service sink for the disposal of mop water and similar liquid waste.

CA: Provided self closing doors in restrooms.

Observed Ceiling in kitchen not smooth, non-porour, non absorbent, and easily cleanable.
CA: Shall install and approved ceiling in the kitchen prep area within 30 days. And 30 more days to install new approved ceiling tiles from the kitchen. Entry to the rear door (dishwashing area)

Observed raw wood exposed at 4 compartment sink. Paint or seal with water proof paint or sealient.
CA: Advised owners to seal all raw wood in kitchen and around sinks.

(a) Indoor Materials. Materials for indoor floor, wall, and ceiling surfaces under conditions of normal use shall be:

Smooth, durable, and easily cleanable for areas where food service establishment operations are conducted;

Closely woven and easily cleanable carpet for carpeted areas; and

Nonabsorbent for areas subject to moisture such as food preparation areas, walk-in refrigerators, warewashing areas, toilet rooms, mobile food service unit servicing areas, and areas subject to flushing or spray cleaning methods.
