Observed employee beverage without lid and straw next to sliced tomatoes on prep table. Advised PIC employee beverages shall be in disposable cup with lid and straw. COS- employee beverage discarded

No paper towels observed @ hand sink. Advised PIC paper towels shall be provided @ all times. COS- paper towels were provided.

Observed Several TCS foods cooling tightly covered. Advised PIC of approved cooling methods.

Observed leak underneath dish machine. Advised to repair.
