Observed multiple pull out couches with dirty plastic sheets hanging from the bottom, room 103 had vomit/mold like substance on the bed bug cover on the mattress, room 201 has a burn hole in the bed bug cover destroying the seal, multiple rooms had stained linens at the time of this inspection. CA: Increase cleaning frequency and retrain staff to ensure bed bug covers are intact and linens are not stained

Observed facility had inspection report and permit hung up where the public would not be able to view them at the time of this inspection. CA: Ensure both are posted where the public can view them easily

Observed rooms 228 and 307 with toilets that did not flush at the time of this inspection. CA: Repair toilets and ensure all toilets are able to flush
