COS: Manager created a sanitizing solution manually which was within the acceptable range (200-400 ppm for Quaternary Ammonium) while they fix the dispenser. Observed several items in disrepair in the facility at the time of this inspection such as the ceiling tiles which were moldy and wet, the A/C vents where dripping large amounts of water, cooling unit in WI cooler was dripping onto a bucket, the hood system's filters were covered in dust and grease and the hot drawer was not working properly at the time of this inspection. CA: Physical facilities shall be installed and maintained in good repair to avoid potential sources of contamination.
