1-2B - certified food protection manager
Regulation: 511-6-1.03(3)(a) - food safety manager certification (pf)
Observed that facility does not have a food safety manager certification at the time of this inspection. CA: Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM) as specified in paragraph (b) of this subsection to ensure food safety is being managed within the food service establishment during all hours of operation.
EH informs owner/manager about rule 511-6-1-.10(a) Inspections and Compliance Procedures and failure to correct violation upon third consecutive inspection. Manager will send proof of previous attempts to take CFSM certification as well as proof of enrollment to take next CFSM examination.

Observed a box with SSI lids stored on the floor at the time of this inspection. COS: Removed from floor. CA: Single service items shall be stored at least 6 inches (15 centimeters) above the floor.
