Observed guest room water temperature over 120F at the time of this inspection. CA: Lower water heater temperature to ensure guest room water temperature does not exceed 120F

Regulation: 511-6-2-.13(1) - insect and rodent control
Observed live roaches in guest room 236 at the time of this inspection. COS: Facility is closing room 236 for treatment by pest control. CA: Facility must have room 236 treated for roaches by professional pest control before returning the room to guest use
8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot

Observed facility did not have a gas-fired inspection at the time of this inspection. CA: Facility must ensure that gas-fired equipment is inspected annually and that a copy of this inspection is available on site at all times

Observed damage to guestroom tub/shower walls at the time of this inspection. CA:Repair shower/tub walls to ensure that water damage behind the wall does not occur

Observed during the course of this inspection that the facility had multiple rooms where the doors leading to the outside were not properly sealed allowing light to come in through the doorframe. CA: Glynn Eh instructs management to ensure that all doors leading to outside areas are properly sealed at all times
33 - housekeeping carts and storage areas maintained; clean/sanitized items protected from contamination

Observed during the course of this inspection that the facility had a housekeeping cart that had chemical and bug sprays stored over clean linens. CA: Glynn Eh instructs management to keep the housekeeping carts properly organized and set up to protect the clean linens at all times, Never put chemicals above lines or where the pose a contamination risk to clean linens or single service items
