Observed PIC unable to demonstrate documentation or state what 5 symptoms and 6 illnesses prevent an employee from working with food. CA: Explaining how the person in charge, food employees, and conditional employees comply with reporting responsibilities and exclusion or restriction of food employees. No COS can be noted. Inspector provided employee health policy and reporting agreements, will return to verify document is understood and signed.
2-2E - response procedures for vomiting & diarrheal events
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
Observed no bodily fluid clean-up kit or procedures. CA: Food service establishments must have written procedures when responding to vomiting or diarrheal event. No COS can be noted. Inspector must return to verify bodily fluid clean-up kit has been purchased.

Observed ice machine chute soiled with black debris. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. No COS can be noted, inspector must return to verify ice chute has been cleaned to sight and touch.

Observed interior of microwave soiled with food debris. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: Employee assigned to clean interior of microwave to sight and touch.

Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed reach-in freezers with excessive ice accumulation. Gasket replacement to ensure door is sealing properly. CA: Equipment shall be maintained in a state of repair and condition that meets the requirements. No COS can be noted. Inspector must return to verify gaskets have been replaced and ice build-up has been cleaned from reach-in freezers.
