NOTE: The sliced mushrooms (GIN: 0074391) do not have any labeling stating that the food is pre-washed. The food is obtained in cardboard boxes with ventilation holes. The letter provided during the inspection states that produce comes in pre-washed (dated May 2017). This letter was reviewed by our State Health Department Office and the Yum Brands Regulatory Department in November 2018. It was then determined that the letter excludes mushrooms. Therefore, the mushrooms must be treated as unwashed (stored below ready-to-eat foods and washed before serving). It is also noted that the GIN has changed since that communication. Documentation must be provided indicating that the food is pre-washed before storing it on top shelving and not washing it before serving. NOTE: An informal follow-up inspection will occur by MARCH 27 after 10 AM to check on the following items: -11D Installing a thermometer in the reach-in cooler, -10D Posting written food allergen disclosure, -13A Posting physical copies of the inspection report, permit, and choking poster, and -13B Posting a physical "no smoking" sign.
