4-1A: .04(4)(n)1Observed person in charge crack raw eggs with gloves, then cut a tomato slice while wearing the same gloves.// If used, single-use gloves shall be used for only one task such as working with ready-to-eat food or with raw animal food, used for no other purpose, and discarded when damaged or soiled, or when interruptions occur in the operation. (P) Corrective Actions: Tomato was discarded. Person in charge removed gloves, washed hands, and put on new gloves.//

4-2B: .05(6)(n)Observed in-use dish machine not reaching proper sanitization temperature. SECOND CONSECUTIVE VIOLATION After being cleaned, equipment food-contact surfaces and utensils shall be sanitized in hot water mechanical operations by being cycled through equipment that is set up as specified under subsections (6)(e), (l), and (m) of this Rule and achieving a utensil surface temperature of 160ºF (71ºC) as measured by an irreversible registering temperature indicator. (P) Corrective Actions: Dish machine was repaired during inspection.
4-2B: .05(8)(a)Observed employee rinse off pan and put it back on line to be used without sanitizing. Utensils and food-contact surfaces of equipment shall be sanitized before use after cleaning. (P) Corrective Actions: Pan was recleaned and sanitized.

SECOND CONSECUTIVE VIOLATION Prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as "keep refrigerated" and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer's expiration date or the sell-by date. (P) Corrective Actions: Items were discarded.
