Repeat violations observed today (14A and 14B). Three consecutive violations of the same code provision will result in a permit suspension. Note: Ensure back door is completely tight-fitting to frame. Note: Allergen notification guidance provided to person in charge. Note: Ensure a handwashing sign is added to handwashing sink at bar. 4-2B: .05(7)(b)Observed person in charge remove thermometer from pocket and attempt to take a temperature of food without sanitizing the thermometer first.// Equipment food-contact surfaces and utensils shall be cleaned before using or storing a food temperature measuring device. (P) Corrective Actions: Person in charge sanitized thermometer after use. Food was discarded.//

14A: .04(4)(k)Observed bowls with no handles being stored in three spices and a container of flour. Observed a scoop in rice with the handle in direct contact with the food. SECOND CONSECUTIVE VIOLATION// During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: 1. in the food with their handles above the top of the food and the container; 2. In food that is not time / temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed; 3. On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized at a frequency; 4. in running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes; 5. in a clean, dry container; or 6. in a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at the required frequency. (C) Corrective Actions: Bowls were removed. Rice scoop was positioned with handle out of the food.//

14B: .05(10)(e) 1,2,4Observed several metal and plastic containers stored as clean and stacked wet. SECOND CONSECUTIVE VIOLATION// Clean equipment and utensils shall be stored in a self-draining position that allows air drying and covered or inverted. (C) Corrective Actions: Containers were washed, rinsed, and sanitized again.//

15B: .05(7)(g)Observed employee place small metal pitchers into a large metal container in the warewashing machine in a position that did not allow the items to drain. After the machine ran, the metal container and small pitchers were filled with solution.// Soiled items to be cleaned in a warewashing machine shall be loaded into racks, trays, or baskets or onto conveyors in a position that:
- Exposes the items to the unobstructed spray from all cycles; (C) and
- Allows the items to drain. (C) Corrective Actions: Items were run through the warewashing machine again in self-draining position.//

17D: .07(4)(b)Observed employees' personal belonging stored next to and above pitchers, liquor bottles, and wiping cloths in cabinet.// Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur. (C) Corrective Actions: Personal items were relocated to a designated area.//
