Have all violations corrected by April 9, 2026 or permit may be suspended. NOTE: Table cleaning procedures were discussed with the person in charge. The spray that was being used per the discussion with the person in charge had in the instructions that the it was not food contact safe.

10D: .04(7)(d)Observed that the restaurant does not have a written consumer notification displayed to the public indicating which food allergens are present in the facility. There is no notification on the front service line or on the drive thru board.// 1. If required by law, consumer warnings shall be provided. (C) 2. Food service establishment or manufacturers' dating information on foods may not be concealed or altered. (C) 3. The permit holder shall notify consumers by written notification of the presence of major food allergens as an ingredient in unpackaged food items that are served or sold to the consumer. (C)

14B: .05(10)(e) 1,2,4Observed containers that were identified as clean wet stacked on the shelf across from the three compartment sink.// Clean equipment and utensils shall be stored in a self-draining position that allows air drying and covered or inverted. (C) Corrective Actions: Person in charge removed the wet stacked containers and had them recleaned and sanitized. The containers once recleaned are allowed to dry completely before stacking.//

17A: .06(2)(h)Observed that the restroom has a hand dryer ( without paper towels) in the bathrooms but does not have a handsfree way to exit bathrooms without either touching the handle or the lock since each are single occupant restrooms. Paper towels were not present in either bathroom.// In toilet facilities that have exit doors with handles or knobs that must be touched to open, sanitary towels must be provided. (C)
