12B: .03(5)(i)Employees involved in food preparation with hair greater than 1/2" not wearing proper hair restraints. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch (1/2"). (C) Corrective Actions: Proper hair restraints provided.

14C: .05(10)(f)Single use articles stored on shelving unit in the employee bathroom. Single use articles are prohibited from being stored in bathroom areas to prevent possible contamination. Corrective Actions: Items were removed from the bathroom.

8-2B: .07(6)(c)Spray bottle of glass cleaner left on prep counter. // Poisonous or toxic materials shall be stored and displayed for retail sale so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles. (P) Corrective Actions: Bottle moved from prep counter to an area where it cannot contaminate surfaces. //
