All cold holding in compliance unless other noted. NOTE: Facility is adding two new food trucks (Travelin Tom's Coffee) under the same corp. Will be applying for two new food truck permits. NOTE: The ice machine in the commissary is no longer in operation. All ice is purchased from an approved vendor. NOTE: The facility uses food additives: All syrups have food coloring for the ice including but not limited to Blue Raspberry, Lucky Lime, Tiger's Blood, Watermelon Wave, Strawberry Treasure, Pina Colada, Monster Mango, Ninja Cherry, Island Rush, Groovy Grape. NOTE: Ensure that ALL single use cups are stored off the floor in the warehouse and in the office.

NOTE: Facility only serves shaved ice, sugar water, and flavored syrups for snow cones. NOTE: Per owner, he has a total of 7 trucks. Per owner, some are used for food service and some are used for catering events. Per owner, the truck that is currently permitted is not in use and is in the process of being sold. Discussed with owner that he needs to decide which trucks will be used for food service and that those trucks will need a food service permit application. Mobile food service permit application provided. Applications for trucks used for food service must be submitted to Gwinnett County's Environmental Health Department within the next 30 days. **Failure to comply may result in food service permit SUSPENSION for the commissary or a citation.** NOTE: Observed commercially packaged food and single use items stored in the garage area attached to commissary. Garage area does not have approved finishes. Discussed with owner that all food items must be stored in the kitchen or trucks. Discussed with owner that single use items must be moved into the kitchen or truck if the seal of the box has been broken. NOTE: Per owner, facility prepares sugar water and that water being used is from the mop sink. Discussed with owner, that water used for food purposes must retrieved from the 3 compartment sink (per area manager ZAG).
