18: .07(5)(o)Small lizard was trapped in the men's restroom sink. Live animals may not be allowed on the premises of a food service establishment (except for fish aquariums, controlled service animals, patrol dogs accompanying security or police officers, pets in residential common areas at times other than meals, and on outside premises when special requirements are met by this Rule). (Pf) Corrective Actions: Lizard was removed.

2-2D: .06(2)(o)Both hand sinks were not operational. Facility is not prepping vegetables and will use that sink to wash hands until they are repaired. A handwashing sink shall be maintained so that it is accessible at all times for employee use. (Pf)

17D: .07(4)(b)Employee items like bag, umbrella, and phone were being stored above sugar. Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. (C) Corrective Actions: Items were moved.
