Observed outer opening backdoor in bakery area with large gap and not self-closing. PIC stated they are actually here today to fix the gap and that they will also make the door self-closing. Observed outer opening backdoor near bathroom area is not self-closing.

(m) Outer Openings, Protected.

1. Except as specified in paragraphs 2 through 5 of this subsection, outer openings of a food service establishment shall be protected against the entry of insects and rodents by: (i) Filling or closing holes and other gaps along floors, walls, and ceilings; (ii) Closed, tight-fitting windows; and (iii) Solid, self-closing, tight-fitting doors.

Observed water damage on the ceiling and walls in dry good storage room in bakery area. PIC stated it happened during the last bad storm; they had to throw away items that were affected and have asked maintenance to fix the damage.

(5) Maintenance and Operation. (a) Good Repair. All physical facilities shall be maintained in good repair.

No sanitizing test strips on premises, or knowledge of the process of using them. The following signs were no posted: no smoking, choking hazard. No knowledge of vomit clean up procedures. Multiple bulk ideas were no properly labeled. Expired food items in back cooler (dip, almond paste). Person in charge not performing duties; not ensuring employees are properly sanitizing food contact surfaces or equipment through routine monitoring of solution temperature and exposure time for hot water sanitizing, chemical concentration, pH, temperature, and exposure time for chemical sanitizing.

Establishment does not have established procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment. Consulted with PIC on methods for responding to vomit events.

Sanitizer not used in accordance with the manufacturer's recommendations.

Health Authority tested concentration on quaternary ammonium sink and the results were 0 ppm. Current testing methods were not being followed according to manufacturer recommendations. New method established to bring the sanitizing sink up to the proper ppm.

Observed opened/prepared container/package of ready-to-eat potentially hazardous food that does not bear a consume/sell by date/label. Food may not be served.

Multiple items observed past 7 week expiration date (spinach artichoke dip and almond paste)

Observed rodent activity as evidenced by rodent droppings found.
