Observed CFSM to be CFSM at a different food service establishment. 511-6-1.03(3)(b) - Certification Requirements/Exemptions (Pf) At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall be a certified food safety manager who has shown proficiency of required information through passing a test that is part of an accredited program that conforms to the national standards for organizations that certify individuals. Certified Food Safety Managers must be designated to one food service establishment only and maintain and renew certification in accordance with the requirements of the examination taken. Note: The facility will need to hire a CFSM or have a food service employee with managerial control obtain the certification.

Observed food employee without a hair restraint, while washing utensils at the 3-compartment sink. 511-6-1.03(5)(j) - Hair Restraints (C) (i) Hair Restraints.
- Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS: The food employee obtained a hat to restrain their hair before returning to working with food and dishware.

Observed food debris accumulation throughout the flooring of the facility from the employee restroom through the main kitchen to the mop area. 511-6-1.05(7)(d) - Nonfood-Contact Surfaces (C) (d) Nonfood-Contact Surfaces. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Note: The facility will need to have the flooring deep cleaned and establish a cleaning schedule for maintenance of nonfood-contact surfaces.

Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered: 2. With tight-fitting lids or doors if kept outside the food service establishment. COS: The side doors of the outside dumpster were closed by the PIC.

(a) Floors, Walls, and Ceilings, Cleanability. Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Note: The facility will need to have this repaired to ensure that these areas are smooth and easily cleanable.
