Observed several food containers wet stacking on the rack across from 3-compartment sink. After cleaning and sanitizing, equipment and utensils:
Shall be air-dried or used after adequate draining before contact with food; and
May not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry. Correct within 72 hours.

Observed debris and grease build-up between several non-contact food surfaces of equipment in main kitchen area. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. Correct within 72 hours.

Observed Floor in kitchen area behind fryers in need of cleaning. 1. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.

Observed an accumulation of soil or mold in and along the nozzles of the beverage dispensing machine. In equipment such as ice bins and beverage dispensing nozzles, equipment shall be cleaned at a frequency specified by the manufacturer or at a frequency necessary to preclude the accumulation of soil or mold. COS:PIC removed nozzles and washed, rinsed and sanitized.

Observed grease buildup and residue on floor and walls of main kitchen. floors were slick and slippery. Except for anti-slip floor coverings or applications for safety, floors, floor coverings, walls, wall coverings and ceilings shall be designed, constructed, and installed in such a way that they are smooth and easily cleanable. CA: PIC ordered non-slip matting to address flooring concerns and had employee clean floor problem areas.

Observed that PIC could not demonstrate in a verifiable manner that food employees are/were informed of their responsibility to report symptoms and/or illness. PIC shall demonstrate knowledge by explaining the responsibility of the PIC for preventing the transmissions of foodborne disease. COS: EHS issued DPH red book (employee health) to PIC.

Observed that the most current inspection report was not posted within facility. The most current inspection report shall be prominently displayed in public view at all times. COS: EHS issued new inspection (current) to PIC.

Observed food debris and other residue on walls and floors of facility. Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed in such a way that they are smooth and easily cleanable. COS: PIC had employee clean walls and floors to remove residue.
