Observed gas-fired equipment inspection not on file for the hotel. Manager will have gas-fired inspection conducted no later than 2/26/26 and email the inspection report to keundra.golden@dph.ga.gov

Room 206: Stains observed on the pullout mattress and mattress cover. Room 216: Observed small reddish/brown on pillow. Room 115: Hair observed on the bedding. Room 111: Observed stains on pullout mattress cover

Observed 2nd, 3rd floor ice machine mold like substances on interior surface of the ice chute

Observed 3rd floor housekeeping room with chemicals stored in spray bottles without identifying labels on the shelf and housekeeping cart

Room 319: Observed the TV stand drawer in bad repair (won't close)

Room 322: Observed a pink, oddly shaped stain on the mattress protector. Observed a small yellow stain on the sofa's mattress. Room 226: Observed a yellow stain on the mattress cover and pad. Room 221: Observed stains on the couch pillow

Room 326: Observed a foul odor in the bathroom

Ice machine on the second floor with black mold like build up

On the 2nd floor observed chemicals not in the proper bottles

Observed no annual gas fired equipment report accessible for viewing at the facility. Facility have 30 days to get all gas-fired equipment inspected.

All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Room 316: Observed a brown debris on AC unit and a earring. Room 311: Observed a stain on the headboard. Room 225: Observed a computer desk with stains and a brown stain in the freezer. Room 218: Observed brown stains in the freezer

Room 326: Observed stains on the fitted sheet and hair on the sheet. Room 316: Observed hair on the sheet and mattress cover. Also, stains on the pillowcase. Room 225: Observed stains on the pillow. Room 218: Observed stains on the pillow, hair on the mattress cover, and stains and hair on the mattress pad on the sofa couch

Room 326: Observed hair on the soap holder in the tub. Room 311: Observed stains on the towels. Room 225: Observed the ceiling with black stains and the shower curtain with an orange stain. Room 218: Observed hair in the tub and stains on the towels. Room 111: Observed stain on the shower curtain

Room 311: Observed debris on the floor behind the bed. Room 225: Observed plastic on the floor behind the bed

Room 326: Observed a small amount of debris in the ice bin. Room 311: Observed brown stains on the exterior surface of the ice bucket.

Observed a pink debris on the ice machine chute on the 3rd floor and a black mold-like substance on the ice machine chute on the 2nd floor. The second floor's ice machine is currently down and has an "out of order" sign on it. Equipment must maintain the cleanliness while it is down

Housekeeping room on the 3rd floor: Wall with a hole that must be sealed and painted. Also, the sink needs to be sealed to the wall

Room 225: Observed hair on several bedding items (sheet, comforter, mattress pad. Also, observed stains on the pillow, pillowcases, and mattress pad

Observed towels in the bathroom with hair and stains. Also observed a black dust-like debris in the ceiling

Observed stains on the wall under the computer desk. Also observed debris on the floor behind the bed (looked like paper)

Observed the ice machine on the second and third floors contained black and pink mold debris on the white chute. Out of order signs were made and placed on the ice machines for cleaning (COS).

Also observed a brown debris on the ice on the top of the reach in freezer in room 225

Observed 2 chemical bottles with no name to properly identify what they are. Also, on the 2nd floor the sink in the housekeeper room needs to be sealed to the wall
